2. A 70 year old woman has progressive low retrosternal dysphagia, initially to solids and now also to liquids.  She complains of chest pain and weight loss over the last 3 months.  A social history reveals that she has been a heavy smoker for many years and drinks around 20 units of alcohol a week.
